Logistics: Transferring patient from King George hospital to Queens hospital took longer than usual. During oozing of wound no one was around the bay area.
Diagnosis: The swab test results were never out. The sample taken for test doesn't have the patient name. Review and inspection should be in place to improve the details are available before send to lab.
